Why are women more affected by varicose veins? The role of hormones, explained by Dr Paul Pittaluga

Over the course of her life, a woman goes through hormonal phases that directly affect the tone, the elasticity and the strength of her veins. This article explains those mechanisms and shows you when to act.
1. Women, the group most affected by venous disease
Chronic venous disease is one of the most common conditions in Western countries. In France, around 18 million people live with some degree of venous insufficiency. But it is far from evenly distributed between the sexes.
Varicose veins are estimated to affect 10 to 20% of women before the age of 40, 20 to 40% between 40 and 50, and probably more than 50% beyond the age of 60. At Paris Veine Institut, women over 45 account for two thirds of the female patients who undergo varicose vein surgery, and the average age at a first procedure is 52.
This female predominance is no accident. It comes down to specific biological mechanisms, tied to the hormonal profile of the female body, which weaken the vein walls at every major stage of reproductive and post-reproductive life.
2. How female hormones act on the veins
Hormone receptors in the vein walls
The inner lining of the veins has receptors specific to the female sex hormones, oestrogen and progesterone in particular. Veins are therefore a direct target for hormonal fluctuations throughout a woman's life.
How oestrogen acts
Oestrogen has a complex effect on the venous system. It can promote sodium and water retention, and increase both capillary permeability and venous distensibility. At high levels, it also contributes to changes in blood clotting, which can increase the tendency to form clots in some women.
How progesterone acts
Progesterone has a well-documented relaxing effect on the smooth muscle cells of the vein walls: it reduces venous tone, meaning the ability of the veins to contract and push blood back towards the heart. As a result, the veins dilate more easily, blood pools in them more readily, and the symptoms of venous insufficiency, such as heaviness and swelling, become more pronounced.

3. The menstrual cycle: heavy legs before your period
Many women notice their venous symptoms getting worse in the days before their period: heavier legs, slightly swollen ankles, a feeling of tightness. This is no coincidence.
At the end of the cycle, during the luteal phase, progesterone levels peak. Venous distensibility is at its highest, venous return slows, and venous symptoms are at their most intense. Once the period starts, the sharp drop in these hormones usually brings rapid relief.
This cyclical pattern is often the first sign that a woman's venous system deserves attention, well before any visible varicose veins appear.
4. Pregnancy: the most critical period for the veins
Pregnancy puts the veins under particular strain. Many women see varicose veins appear, or find that existing symptoms get worse.
Contrary to a common belief, pregnancy alone does not cause varicose veins. According to Dr Paul Pittaluga, it usually reveals a pre-existing venous weakness, very often a hereditary one.
The explanation is primarily hormonal. During pregnancy, oestrogen and progesterone levels rise sharply, and both hormones promote vein dilation. In women whose vein walls are more sensitive to these hormonal shifts, the veins dilate more easily and the symptoms of venous insufficiency can intensify.
Not every woman reacts the same way, however. Some develop varicose veins during their first pregnancy, while others only do so after several pregnancies, or even several years later. The risk rises with each pregnancy: it is estimated at 23% for a first pregnancy, 27% for a second and a third, then 31% from the fourth onwards, particularly where there is a hereditary predisposition.
Heavy legs, swelling and venous pain are also more common during pregnancy. The risk of a blood clot increases as well, especially in the weeks following delivery.
After the birth, some veins gradually return to a normal appearance as hormone levels fall. Others stay permanently dilated and become true varicose veins.
During pregnancy, treatments to remove varicose veins are contraindicated. Management relies mainly on properly fitted compression stockings, on walking, and on measures that support venous return.
5. Menopause: a turning point for vein health
Menopause occurs on average around the age of 51 in Western countries and marks the end of the reproductive hormonal cycles. Its impact on venous circulation is paradoxical, and often poorly understood.
An apparent paradox: fewer symptoms, but varicose veins that keep progressing
The natural fall in oestrogen and progesterone at menopause tends to reduce venous distensibility and symptoms such as heavy legs and swelling. Some women report that their legs trouble them less after menopause than before.
But this apparent relief masks a more worrying reality: varicose veins carry on progressing, structurally and silently. Age damages the vein wall, the valves deteriorate, and the risk of complications, including thrombosis, skin changes and venous leg ulcers, increases over the years.
What menopause actually changes for the veins:
- Silent progression: varicose veins spread with age, even in the absence of significant pain
- Higher clotting risk: the risk of a blood clot rises naturally with age, independently of hormones.
- Associated factors: weight gain, osteoarthritis and reduced activity caused by pain all make matters worse.
6. Hormone replacement therapy: should you avoid it if you have varicose veins?
Many women ask the same question at menopause: are varicose veins a reason to forgo hormone replacement therapy (HRT)?
For Dr Paul Pittaluga, the answer is clear: in the vast majority of cases, no.
Outside specific situations, notably a personal history of breast cancer or a high risk of blood clots, having varicose veins should not lead a woman to forgo hormone replacement therapy when it is indicated.
Hormones can influence how the veins work and contribute to symptoms in women who already have a venous weakness, but that effect has to be weighed against the benefits the treatment brings to the body as a whole.
In practice, Dr Pittaluga recommends a broader approach: monitor the venous system regularly, treat varicose veins when they need treating, and do not forgo hormone replacement therapy when it is well indicated, given how substantial its benefits are.
In other words, this is not a choice between treating your menopause and protecting your veins. With the right medical follow-up, the two can be managed together.

“Hormone replacement therapy should not be avoided simply because someone has varicose veins. Outside specific situations, notably a personal history of breast cancer or a significant risk of blood clots, the benefits of this treatment for the body are often far greater than any impact it may have on the veins. That said, it does mean having your venous circulation checked regularly and treating your varicose veins when they need it.”
